This webinar acquaints participants with a pilot project involving seven supportive housing providers, that will determine which services help our clients to: remain in housing, access outpatient healthcare services when appropriate, and enroll in public benefits. Because the project involves several agencies, data entered into HMIS needs to be defined and coded the same way across agencies. You will learn the shared service definitions and the HMIS terms and codes for these services, which will allow us to more accurately compare services provided and subsequent client outcomes.
This training is specifically for case managers, supervisors and data managers at agencies participating in the Health Service Model Project for PSH Providers rolling out July 1, 2019.
