    The Funding Specific Enrollment forms list all the necessary universal data elements and custom CoC elements for the following projects:

    • HUD CoC and ESG funded
    • HOPWA funded
    • RHY funded
    • PATH funded
    • VA funded

     

    We encourage agencies to add questions or manipulate these forms in a way that best fits the workflow of the agency, particularly if servicing multi-person households.
