The purpose of this article is to provide eligibility criteria and guidance for Special NOFO-funded Street Outreach Projects.
The following criteria is applicable for the following projects:
- (1922) ReVive Center for Housing and Healing - Street Outreach
- (1866) Thresholds, Inc. - HOP - Westside Unsheltered
- (1850) Featherfist - Targeted Crisis Outreach
- (1929) Street Samaritans - Community Care and Connection
- (1939) Renaissance Social Services, Inc. - Homeless Outreach Team
- (1918) Puerto Rican Culture Center - Outreach and Engagement
- (1859) The Night Ministry - Street Medicine Program
SNOFO Street Outreach projects should still follow the standard Chicago-CoC workflow: Street Outreach/PATH Workflow - 2023 Update. However, it is the provider's responsibility to determine eligibility before service begins and a project enrollment is entered into the HMIS.
Participants must be experiencing unsheltered homelessness, as defined by HUD, at first contact in order to be eligible.
According to the Street Outreach/PATH Workflow, this is documented as a Current Living Situation entry.
Eligible participants must be first contacted in a situation that falls under and is documented in their entry assessment as “Place not meant for habitation (e.g., a vehicle, an abandoned building, bus/train/subway station/airport or anywhere outside) (HUD)”
If a participant has been already serviced but was later determined ineligible, they must be exited from the project.
The provider may be required to explain these instances in their QPR/APR.
According to the CFR, supportive services only be provided for up to 6 months after the household moves into housing.
According to the Street Outreach/PATH Workflow, this is documented as a Project Exit.
- Exit date must reflect the last date the participant receives services.
- Destination must reflect their Living Situation upon being exited.
