New Agencies must complete the following steps in order to receive access to the HMIS. The requested information should be sent to the Help Desk at helpdesk@allchicago.org.
- Confirmation that your agency is eligible for the use of the HMIS:
- Must have funding that requires the use of the HMIS. Please provide us with your funding stream and grant number, indicating if you are also a sub-recipient of All Chicago
- Must be located within the IL-510 CoC. Please provide us with the main address of the organization.
- New agencies must complete the Agency Participation Agreement before getting any HMIS access. Please copy or provide us with the Executive Director's contact information so that we may send them the APA.
- The agency itself and participating projects will then need to be created in the HMIS. Please review and complete the form found in this article: New Project Creation
- Users will need to complete training before getting live access to the HMIS: Steps of Training and Receiving HMIS Access
- Provide a list of users that will need HMIS access with the following information:
- Name
- Agency-Specific emails
- User-created LMS username
- Projects they will need access to
- Expected responsibilities within the HMIS
