Coordinated Entry Standardized Housing 2024 Assessment
Overview
The global Client Contact Information sub-assessment originated from the HMIS committee giving authorization for the HMIS Team to find ways to better share Client Contact information throughout the system and across agencies. The solution was to leverage the Client Profile Tab so that contact data would be available for all agencies even if an enrollment does not exist. We also were able to leverage HPCC and Coordinated Entry assessments since contacting the client is an important factor in enrolling referred clients. The Client Contact Information sub-assessment is designed to be a universal way that contact information for clients can be globally shared throughout the system. This sub-assessment can be completed for all clients newly added to the system or have already been created. This sub-assessment allows you to add as many entries to this field and the history will remain visible. This will allow agencies to see all contacts the client provided over time no matter the agency that entered the data.
Location
This sub-assessment is a part of the Client Profile Tab, HPCC 2024 Assessment, and Coordinated Entry Standardized Housing 2024 Assessment. This sub-assessment can not be found on any other location or assessment currently, including the HUD and DFSS assessments. All users from any agency can access this sub-assessment by viewing the Client Profile tab under their agency ID number. However, users who are using EDA mode will not be able to view this sub-assessment at the moment. If your project receives client referrals from Coordinated Entry or HPCC, you can use this assessment to contact clients for Permanent Supportive Housing (PSH) or HP.
HPCC 2024 Assessment
The Client Contact Information sub-assessment can be located via the HPCC 2024 Assessment by navigating to the HPCC (644) enrollment. The sub-assessment will be the first question on this assessment.
Client Profile
You can find the Client Contact Information sub-assessment under the Client Profile tab. All users who view clients under their agency ID can access this. The sub-assessment can be used by agencies to document or update client contact information whenever required. This information is shared globally across the system. Visibility for locked records will remain unchanged.

- On the Client Summary page, Click the Client Profile tab
- The Client Contact Information sub-assessment can be found in the 3rd table from the top
Coordinated Entry Standardized Housing 2024 Assessment
The CE assessment now includes the Client Contact Information sub-assessment, replacing the existing contact questions. The existing contact questions have been marked in red on the assessment and only remain on the assessment for reference. No Data Should Be Entered Into The Red Retired Fields. These existing contact questions will remain on the assessment until March 1st, 2024.
Existing 1474 Enrollments
Users completing updates to clients should transfer over previously entered contact information into the Client Contact Information sub-assessment so that information will not be lost after March 1st, 2024. Do Not Add New or Updated Data Into The Red Retired Fields.

New 1474 Enrollments
