You can easily keep track of client goals and case notes in HMIS. Go to your client's profile and navigate to the Case Plans tab. In order to add a case note, you will need to add a corresponding goal.
First, go to the Case Plans Tab. Then click the "Add Goal" button. This will cause the Add Goal pop-up to appear:
From this pop-up, you will:
- Select Household Members included in the Goal
- Select your Provider (Project)
- Choose the Date this goal was set
- Choose a Classification and Type (e.g. Classification: Housing, Type: Move to Permanent Housing)
- Set the Overall Status (e.g. Identified, In Progress, etc.)
After filling out the Goal information, click the Add Goal button, and further options will appear:
You now have the ability to add Case Notes, Action Steps Planned, and Service Items for the Goal. To add, click the Add Case Note button and a new pop-up will appear.
Note: Be sure to add a Case Note before adding Services. Adding a Service will navigate you away from this pop-up. If you do not want to add Services, scroll down and click Save & Exit to complete.
Complete the required information and click Save Case Note. This will bring you back to the previous Goals pop-up.
To add a case note, you will just need to select the household members, choose your provider (Project), set the appropriate date, and type in your notes. Then click the Save Case Notes.
If you are adding Services related to the Goal, then click the Add Service button on the Goals pop-up. This will cause a warning to pop-up notifying you that you are being redirected to a different Tab in the client profile. Select OK, and complete the Services information.
