The Chicago CoC requires that all organizations using the Homeless Management Information System (HMIS) complete and execute an Agency Participation Agreement (APA) annually. We renew these in January of each year. We must have a signed agreement each year for your agency to continue to access the HMIS. The Agency Participation Agreement sets forth the responsibilities of each organization regarding the use of the HMIS as well as those of All Chicago Making Homelessness History as the HMIS Lead. Since the terms and conditions have been set by the CoC, we are not able to modify the agreements on an agency-by-agency basis.
