Participants enrolled in EHI Rapid Re-housing Projects are eligible to access services designed to help them increase their income through engagement with an employment or a SOAR-trained benefits advocate. This tool will guide you through the process for making referrals to SOAR-trained benefits advocate.
After completing an intake assessment into your project, scroll to the top of the intake and select the SOAR Screening Tool.
Complete the SOAR Screening Tool below. If the client identifies health problems and issues with work, and wants to seek SSI/SSDI benefits, make the referral to (1799) All Chicago - SOAR (see Referral section below).
TIPS:
If a client identifies that a SSA application is already pending and they have an existing advocate, confirm the client knows how to make contact with that person and encourage them to keep in contact.
If you can’t figure out the status of client’s current benefits claim, or client can’t reach their legal advocate or there is uncertainty and client wants help with SSI/SSDI, make a referral to one of the SOAR advocate agencies listed below.
Creating a Referral to (1799) All Chicago - SOAR
From the client profile, click on Service Transactions.
Click Add Referrals
Select SSI Applications (NL-1000.8100-820) from the Service Code Quicklist and hit Add Terms.
Right below is the Referral Provider Quicklist. Click the dop down and select (1799) All Chicago - SOAR and hit Add Provider.
Scroll down to the bottom of the page and check the box under Referrals called SSI Applications.
Finally, Select Save ALL.

Send your client’s contact info, the HMIS ID, and your contact information.

Once you have completed the screening tool, documented the referral in HMIS, and contacted either Legal Council of Health Justice or Volunteers of America by email or phone, the referral is complete.
