Participants enrolled in EHI Rapid Re-housing Projects are eligible to access services designed to help them increase their income through engagement with an employment or a SOAR-trained benefits advocate. This tool will guide you through the process for referring your participant to an employment navigator.
After enrolling your client in your program and completing your intake assessment, click on the Employment Navigation Pre-screener.
Complete the assessment below by following the instructions within the assessment.
At the end of the assessment, you'll find some instructions to complete an external referral.
Completing a Service Transaction
1. Click on Service Transactions within the client record.
2. Click Add Service
3. Select an End date of the same date
4. Select Employment from the drop down for Service Type and hit Save & Continue
5. On the next page, scroll down to the bottom and hit Save & Exit.

Send your client’s contact info, the HMIS ID, and your contact information.
Once you have completed the screening tool, documented the referral in HMIS, and contacted either Chicago City Literacy Coalition or Greater West Town by email or phone, the referral is complete.
