The Referrals Provided data element is required for both PATH Street Outreach and PATH Services Only projects. Below is the HMIS workflow designed to identify the referrals that are made on behalf of a client during project enrollment. This guide picks up after the client has been successfully enrolled in the PATH project.
Any service for which a referral is made to another agency, or to another program within your agency not funded or operated by PATH, will need to be documented as a Referral. This documentation is required whether or not the referral was successful in connecting the PATH participant to the service.
The user creating the referral must be in Enter Data As mode under the agency's PATH project.
After clicking on Enter Data As mode, select the PATH project by clicking the Green Plus.
To add a referral, go to the Service Transactions tab of HMIS and click Add Referrals:
To create a referral, you will need to identify the Need Assignment Service Code.
Based on the Type of Referrals provided to the client, the Need Assignment Service Code must match based on the list below. Once selected, click Add Term.
If the referral is made for a service that is not on the list above, it will not be recorded in the PATH report.
Next, select the Project that will provide the service for the referral. If the service will be provided by your agency's PATH project, select your agency's PATH project that the client is currently enrolled in. Click the Green Plus next to the appropriate project.
Under Selected Providers, you will see the project that was selected.
Next, complete the Type of PATH Referral and Outcome listed under the Referral Data section. The Type of PATH Referral must match what was selected for the Needs Assignment Service.
If you are awaiting action on the referral, select Need Status as “In Progress” and select Outcome as “Service Pending.” If the referral has been denied, select Need Status as “Closed,” and select Outcome as “Not Met”. If the referral has been acted upon, select Need Status as “Closed” and select Outcome as “Fully Met.”
Click Save ALL to save the referral.
