The goal of diversion is to rapidly resolve the client's housing crisis in order to divert them from entering the homeless services system. This could potentially include helping provide conflict resolution to the client and whomever they are currently residing with or providing some funds to make it possible for that client to stay in their current housing situation.
When a client comes to an agency for assistance, the first thing that should be completed is the diversion assessment. This assessment is critical in helping to ensure the client is able to avoid entering into the homeless services system.
Entry
To track diversion results in HMIS, agencies funded to divert clients will have to complete the following assessments:
- The HUD CoC & ESG Entry Assessment
- The Diversion Assessment
In order to completely enter a client accurately, you must do the following:
- Search for the client in HMIS (guide to searching is here)
- If a client is not found, then create their record (guide to creating records here)
- Document the client's consent for themselves and their dependents (guide to doing so here)
- Lock the client record if necessary (guide to locking here)
Navigate to Entry / Exit
Select Add Entry/ Exit
Select your Provider and HUD
Press Save & Continue
Next, complete the HUD CoC & ESG assessment
Complete the information above as you normally would. This assessment holds all the information relating to our clients and their current situation.
After completing the HUD CoC & ESG assessment press Save.
The Diversion Assessment would then need to be completed as well
Navigate back to the top of the assessment to the "Select an Assessment"
Click on Diversion Assessment
After completing the Diversion Assessment press Save & Exit.
Service Transactions
Navigate to the Service Transactions Tab
Click Add Service
Next, select the appropriate Provider
Followed by selecting Service Type
If unable to find an appropriate service from the dropdown menu that fits the services provided to the client you can find the full list by clicking Look Up
once completed click Save & Continue
Click the arrow next to Apply Funds for Service
Next change Vendor to the most appropriate service provided to the client.
Under Funding Sources click Add Funding Source
Select Diversion as the funding source
Add the Amount to the Diversion line (Always leave Client Co-Pay Empty)
Finally, change Need Status and Outcome of Need
Click Save & Exit
Exit
Navigate to Entry / Exit tab
Select the Pencil next to the Diversion Project Exit Date
Change Reason for Leaving question
Change Destination to most appropriate situation
Press Save & Continue
Next, complete the HUD CoC & ESG Exit assessment
Complete the information above as you normally would. This assessment holds all the exit information relating to our clients and their current situation.
After completing the HUD CoC & ESG Exit assessment press Save.
The Exit Diversion Assessment would then need to be completed as well
Navigate back to the top of the assessment to the "Select an Assessment"
Click on Exit Diversion Assessment
After completing the Exit Diversion Assessment press Save & Exit.
Follow-Ups
Follow-ups can only be entered after a client has been exited
Navigate to Entry / Exit tab
Click on Follow-ups under the Diversion Project
Click Add Follow-up Review
Select the Follow-up Type under Follow-up Review Type
Press Save & Continue.
In the Select an Assessment section, click Diversion Follow-up
Complete the short Diversion Follow-up assessment.
Press Save & Exit.
