Family Vulnerability Scoring

1. One point for meeting the chronic homeless definition
2. One point for each child under five (pregnancy counts as a child under five) and half
   point (0.5) for every dependent child five or older
3. Two points for any family separation (question #2)
4. Two points for any current involvement with the child welfare system (open neglect
   case, or have a child in foster care) (questions #4 and #5)
5. Add an additional two points if the respondent has ever had a child in foster or
   adoptive care (question #6)
6. One point if female respondent had her first child at age 20 or younger (question #7)
7. One point for experiencing homeless more than three times In the last three years
   (question #8)
8. Two points for any indication of a current or past mental health problem (questions
   #9, #10, and #11)
9. Two points for any indication of a current or past substance abuse problem
   (questions #12 and #13)
10. Two points for parent having a serious medical condition (question #14)
11. Two points for each child with a serious medical or behavioral health problem
    (question #15)
12. Two points if any items in "As a child", were any of the following statements true for
    you?'" are checked (question #17)
13. Add an extra point if parent was in foster care as a child (question #17)
14. SUBTRACT one point if the answer to this question (As a child, were any of the following statements true for you?) is "a little" and subtract two points if it is "not at
    all" (question #17a)
15. Two points if any items in the question "As an adult, are any of the following
    statements true for you?'" are checked {question #18)
