Self-matching is allowed for the SSVF Rapid Rehousing Program only. In cases when the SSVF staff member self-matches the Veteran to their SSVF Rapid Rehousing program, they will complete the self-referral in HMIS and update the need status the same day the assessment is completed. Before completing the following workflow to match a Veteran to your SSVF project, ensure that a CES assessment has been completed with the Veteran and that the Veteran’s VI score falls between 1-0 for Individuals and Youth, and 5.5-0 for Families.
This article will show the steps to Self Matching a client from CES back to an SSVF project.
Only After entering a client into CES via "Enter Data As" mode by clicking the green circle next to the Chicago Coordinated Entry System (CES) - Skilled Assessors Project, can an SSVF project self match a client back to themselves.
Once the assessment is completed, click Service Transactions
Followed by Add Referrals
Navigate to Needs Assignment sub-assessment and select At Risk/Homeless Housing Related Assistance Programs by clicking Add Terms
Click the arrow next to Search for Provider
Under Search Results select the green plus next to the appropriate SSVF project
Confirmation of the SSVF project chosen will be under Selected Providers
Navigate to the bottom of the screen to Need Status and select the most appropriate CES status
Need status must come from one of the CES options only
Then select Save All
After the referral has been completed to the appropriate SSVF provider, exit from "Enter Data As" by clicking the "Chicago Coordinated Entry System (CES) - Skilled Assessors Project" next to Enter Data As mode.
Navigate back to your client's profile and select the Service Transactions tab
followed by View Entire Service History
Click the Referral Tab from the list of tabs
Select the Pencil next to the SSVF Project that says Referral
Navigate to Referral Data and select Accepted
Next, navigate to Need Status and Outcome
Select the appropriate CES Need Status
Followed by the Outcome of Need
Then select Save All
